There are many options for dark circles under the eyes or a prominent tear trough. There are options from skincare, laser, and filler which are non-surgical to surgeries such as lower lid blepharoplasty with orbital fat pad repositioning or injection of fat grafts. Each option has positives and negatives. Orbital rim implants are theoretically permanent but risk extrusion of the implant. Fat grafting is a wonderful method to fill the tear trough and help to give a natural, rejuvenated youthful look. Fat grafting can also help with dark circles of the lower eyelid. Filler (hyaluronic acid such as Juvederm or Restylane) is temporary usually only lasting about a year but fat grafts are theoretically permanent and do not have the risk of encapsulation or extrusion like an orbital implant would. In general, a patient can begin using sunscreen after eyelid surgery once the incision has closed with enough strength to allow some rubbing and massage. This is usually 2-3 weeks for most patients. Prominent tear trough is caused by the osteocutaneous ligament (tear trough ligament). This ligament is found on the maxilla (cheekbone), and between the palpebral and orbital portions of the orbicularis oculi muscle (a circular muscle that surrounds the eyelid). It extends from medial canthus to mid-pupillary line, from there it continues on as the orbicularis oculi retaining ligament. This area can cause a tired or aged appearance of the eyelid as fat descends or atrophies with aging. It can be filled with fillers like Juvederm or Restylane (which are temporary lasting about a year) or with fat grafting (more permanent).
