At the end of your consultation, you are able to schedule your surgery or another appointment depending on your specific needs.

Preoperative gynecomastia appointment

Before your surgery day, Dr. Azouz will often have you return to the office for a preoperative appointment. During this appointment, any last-minute questions you may have can be addressed and your pre-operative photos will be taken. An Azouz plastic surgery team member will go over specific pre-surgery guidelines such as what time you have to stop eating and drinking the night before your gynecomastia surgery.

Gynecomastia surgery day

On the day of your gynecomastia surgery, you will arrive at the accredited facility at your scheduled time. You are wearing loose-fitting comfortable clothes with a top that buttons or zips on the front. You will be checked in and the pre-operative checklist with the nurses will be completed. Dr. Azouz will come to see you for preoperative gynecomastia surgery marking, once again see if you have thought of any last-minute questions and introduce your anesthesia provider. Soon after you will be taken to the operating room and before you know it you will be in recovery. Some patients do not remember the recovery area due to the aesthetic medication given to them during surgery. While at the facility, while you are recovering post-operative instructions are given to the person who is staying with you for the rest of the day. After a while, you are able to go home.  Dr. Azouz and/or an Azouz plastic surgery team member may call to check on you later in the day.

Do I have Gynecomastia?

Gynecomastia is a condition that affects males at any age. Most men report that they have had male breasts since puberty. Gynecomastia can occur due to a glandular issue, stubborn fat, or an imbalance of hormones. It presents as female breast tissue, stubborn fat, and enlarged areolas. A simple test for Gynecomastia involves pinching the areolar and nipple and feeling for rocky, spongey, or fatty tissue in the breast.

Gynecomastia is also known as:

  • Gyno
  • Male gyno
  • Male breasts
  • Man boobs
  • Moobs

How can you tell you have Gynecomastia?

Gynecomastia is thick tissue underneath the nipple/areola complex. This thick gynecomastia tissue can feel like small rocks underneath your skin. An easy at-home test to see if you have Gynecomastia is to pinch the skin of your abdomen and then pinch the nipple/areola complex. If you feel harder tissue in the chest area, you potentially have Gynecomastia.

What’s the best way to confirm Gynecomastia?

Gynecomastia can be confirmed on a physical exam by a qualified health care provider such as board-certified plastic surgeon Dr. Azouz. Dr. Azouz can diagnose Gynecomastia by performing a history and physical exam for confirmation. Experienced plastic surgeon Dr. Azouz can also perform virtual consultations to diagnose and make plans to treat Gynecomastia. Virtual consultations with securely emailed photography and video chat allow Dr. Azouz (in most cases) to be able to confirm the diagnosis of Gynecomastia. With that being said, some healthcare providers who do not have a high volume of Gynecomastia patients in their practice will have you undergo a mammogram or ultrasound to confirm the diagnosis of Gynecomastia.

How do I treat Gynecomastia?

Gynecomastia most often requires surgical intervention to remove the thick, stubborn tissue and fat in your chest. You may have noticed that your Gynecomastia remains present even with diet and exercise. Dr. Azouz performs high volumes of gynecomastia surgery in Dallas, Texas, on a weekly basis for gynecomastia patients all over the country.

Gynecomastia surgery in Dallas with board-certified plastic surgeons Dr. David Azouz and Dr. Solomon Azouz also addresses the removal of armpit fat.

How is Gynecomastia surgery performed?

Dr. Azouz uses a combination of custom-made liposuction cannulas to remove fatty tissue surrounding the chest and lateral chest wall (armpit area). Dr. Azouz also directly excises thicker tissue from the breasts during gynecomastia surgery. This is all done through a tiny incision that is camouflaged in the nipple/areola.

What kind of anesthesia is used during Gynecomastia surgery?

In most cases, male breast reduction surgery with Dr. Azouz is performed under a combination of local anesthetic and sedative medications. This means that you are breathing on your own for your procedure unlike when general anesthesia is used and a machine breathes for you. Dr. Azouz finds that his patients have a quicker recovery with less grogginess and post-operative nausea with the use of sedation and local anesthetic.

What is recovery like for Gynecomastia surgery like with Dr. Azouz?

Gynecomastia surgery performed by Dr. Azouz is done as an outpatient surgery which means you go home the same day as your surgery. You will leave surgery wearing compression garments. It is suggested that you take the rest of the day after your gynecomastia surgery to relax and recover. Many of Dr. Azouz’s gynecomastia patients return to their desk duties within the first few days after surgery. You will have post-operative appointments with Dr. Azouz in his Dallas-based location or with virtual appointments. During each of your follow-up appointments, Dr. Azouz will monitor your healing progress and will guide you back into more strenuous activities such as heavy weight lifting or exercise.

How do I know my grade of Gynecomastia?

Gynecomastia is a condition that can occur in men of all ages. Fat is trapped and intermixed with firm breast tissue with any amount of Gynecomastia. With diet and exercise, the firm breast tissue (Gynecomastia) makes it very hard to lose. This unusually dense breast tissue can distort the chest and lead to unwanted aesthetic changes. Some patients can have considerable breast tissue, and their skin can sag. Gynecomastia is graded according to skin elasticity and the amount of firm breast tissue.

A highly skilled plastic surgeon can diagnose, grade, and treat Gynecomastia in all age groups. Dallas Plastic Surgeons Dr. David Azouz and Dr. Solomon Azouz are known for their techniques to correct Gynecomastia in primary and revision gynecomastia surgery in Dallas, Texas, for men of all ages and body types.

Determining the grade of Gynecomastia

Board-certified Dr. Azouz explains what Gynecomastia of varying grades looks like below.

Gynecomastia, Grade, Determining Scale,

Grade one: “mild” Gynecomastia without sagging

In mild cases, the chest looks normal and does not have excess skin. Some grading scales quantify the amount of Gynecomastia in grams, but this isn’t easy to do before the time of surgery. Therefore, most plastic surgeons define Grade one Gynecomastia as a slight or “mild” enlargement of the breast glands without skin excess.

Grade 1, Gycomastia, Dr. Azouz, Dallas, TX

Grade two: “moderate” Gynecomastia with or without some sagging

When a patient has moderate Gynecomastia but still does not have skin excess, they are usually deemed a grade 2.   This means they have more breast tissue than a mild grade one but do not have breast sag. Some plastic surgeons break upgrade twos into 2a and 2b, which means that 2bs have a minor skin excess. Regardless of the grading system, most plastic surgeons label a grade 2 as those with a “moderate” amount of Gynecomastia.

Grade 2, Gycomastia, Dr. Azouz, Dallas, TX

Grade three: “severe” gynecomastia with mild sagging

When a patient has a severe amount of Gynecomastia but still does not have extreme skin excess, they are usually deemed a grade 3.   This usually means they have more than 500 grams of fat and breast tissue within their chest.

Grade 3, Gycomastia, Dr. Azouz, Dallas, TX

Grade four: “severe” gynecomastia with “severe” sagging

When a patient has a severe amount of Gynecomastia and sagging usually deemed a grade 4, this usually means their breasts sag as a women’s would when she had large sagging breasts. A patient with grade four gynecomastia would usually have severely enlarged areolas pointing towards the floor.

Grade 4, Gycomastia, Dr. Azouz, Dallas, TX

How Can I Tell If I Have a Gynecomastia?

Early signs may include a firm or fatty tissue in the breast or a feminine appearance to the breast or areola. As the condition progresses, some patients may notice sagging to their breasts with contours that resemble a woman’s breast. Men who are dissatisfied with the look or feel of their chest should seek evaluation and treatment by a board-certified plastic surgeon who performs a high volume of gynecomastia surgery, such as Dr. Azouz.

How Common is Gynecomastia?

Overall, research shows that Gynecomastia is very common and may affect as much as seventy percent of men at some point in their lives. There are different levels of severity ranging from almost unnoticeable to severe with sagging of the breast and female appearance. Regardless of age, gynecomastia surgery can be beneficial to patients of all body types and severity.

Can I Prevent Gynecomastia?

In many patients, Gynecomastia is unavoidable and genetically inherited. Avoiding the triggers of Gynecomastia is essential even after surgery to prevent a recurrence.

What Causes Gynecomastia?

The cause of Gynecomastia varies from person to person. There are several accepted theories on what causes Gynecomastia.

The most common cause is usually genetics. When patients think of their family history, they often recognize a family member with a similar body type who also had Gynecomastia.

There are many different drugs and medications that can cause and contribute to Gynecomastia. These include:

  • Anabolic Steroids
  • Antiandrogens (Finasteride)
  • Antimicrobials (Isoniazid, Ketoconazole, Flagyl)
  • Antireflex drugs (Cimetidine, Omeprazole)
  • Cardiac Drugs (Calcium channel blockers, Captopril, Digoxin, Methyldopa, Reserpine, Spironolactone)
  • Psychiatric Drugs (Diazepam, Thioridazine, Tricyclic antidepressants)
  • Illicit Drugs (Alcohol, Amphetamines, Anabolic steroids, Opioids, Heroin, Methadone, Marijuana)
  • Drugs for HIV/AIDS (Antiretrovirals)

Medical conditions can also cause Gynecomastia. These include

  • Medical problems:
  • Androgen resistance syndrome
  • Klinefelter’s syndrome
  • Hyperthyroidism
  • Hyperprolactinemia
  • Kidney Disease
  • Obesity
  • Cancer

Some cases, however, do not seem to have a genetic link or identifiable risk factor.

Even in patients with no risk factors, some patients will still have Gynecomastia.

What Does Gynecomastia Feel Like?

Depending on the severity, Gynecomastia usually presents as firm, spongey or lumpy tissue underneath the areola. Gynecomastia can cause a lack of confidence and poor aesthetics in men at various severities.

How Can I Get Rid of My Gynecomastia?

Gynecomastia surgery is the most effective treatment to get rid of Gynecomastia. This procedure involves liposuction and removal of the thick breast and fatty tissue.

Gynecomastia can be very frustrating as many patients report having it for many years, beginning in puberty. Most patients report being unable to improve their Gynecomastia despite effective workouts and dieting.

Five Ways to Prepare for Gynecomastia Surgery

Gynecomastia Surgery is the correction of enlarged breasts in men that deliver definitive results. For many men, gyno can be difficult to live with since it can interfere with feeling comfortable in your favorite shirts or during intimate moments. If you do not know where to start, but you are ready to do something about enlarged male breasts, then follow these recommendations to enjoy a flat and sculpted chest with gynecomastia surgery.

Adjust Your Lifestyle

Lifestyle could be one of the causes of Gynecomastia. If you don’t currently have an exercise regimen in place, now is an excellent time to start. Make sure to increase your intake of fruits, vegetables, and protein. Also, limit refined sugar and alcohol to occasional treats. Finally, make sure you get at least thirty minutes of cardiovascular exercise three times per week and begin doing resistance training. Losing weight can help your gynecomastia subside and can also help you have healthy habits in place to maintain your results after surgery.

Choose a Gynecomastia Surgeon

Which surgeon is best for Gynecomastia? Your plastic surgeon should be board-certified, experienced in performing gynecomastia surgery, and able to make sure you feel comfortable and heard. Start your search online, look for testimonies, and before and after pictures. Quality is more important than price. Reach a plastic surgeon with a great reputation in Gynecomastia, compare techniques, and decide which surgeon can help you to achieve your goals.

Schedule your first Gynecomastia Consultation

The first consultation is the most important step to ensure yourself a plastic surgeon can help achieve your aesthetic goals. Make sure to bring a list of questions to your consultation and be prepared to discuss your medical history and lifestyle. Once you are being examined by the plastic surgeon, he or she will determine any underlying causes of gynecomastia and help you manage them accordingly.

Always seek out before-and-after pictures from your plastic surgeon to see what kind of results you can expect. Additionally, talk with your physician about your prescription and recreational drug use – these can sometimes cause hormone fluctuations that result in enlarged breast tissue.

Plan Your Gynecomastia Surgery

There are a few different causes of gynecomastia that can all manifest differently. That means your Gynecomastia surgery will need to be customized to your specific needs based on your board-certified plastic surgeon’s recommendations. For example, gynecomastia can be caused by loose skin, enlarged breast tissue, excess fat, or a combination of any of these factors. This is an important determination because gynecomastia surgery can be completed using the only liposuction when fat is a concern. If breast tissue or excess skin is a concern, you may need surgical removal or a combination of both.

Prepare for Recovery

Your recovery will be the most important part of your Gynecomastia surgical process. You can expect to take about one week away from work and regular activities, meaning you should plan ahead for things like time off, meal prep, and arranging for help with any housekeeping, pets, or children. You can generally return to light exercise at one month and will be instructed to wear a specialized compression vest during this time.

What’s new in Gynecomastia Treatment?

One of the newest cosmetic surgery trends is more men seeking improvement in their chest’s look through Gynecomastia treatment. Years ago, men thought that the best treatment for male breasts was to lose weight. Now, more men are discovering that cosmetic surgery provides the sculpted, masculine chest they desire.

Gynecomastia surgery with Dr. Azouz is a day surgery and involves an enhanced recovery protocol meaning most patients describe recovery as pain-free.  Local blocks, specialized medications, and fluids are used to minimize pain.  Dr. Azouz also performs male breast reduction via a drainless procedure, with camouflaged scars and a minimally invasive technique.  This avoids the large scars that can be an obvious sign that cosmetic surgery has been performed.

Gynecomastia Surgery l Dallas l Dr. Azouz

Dr. Azouz has also created customized liposuction cannulas used specifically in gynecomastia surgery.  These cannulas allow for the removal of both glandular and fatty tissue with high-definition body sculpting techniques.   Dr. Azouz can also directly excise the gland or “breast bud” via a small camouflaged incision.   Dr. Azouz explains that removing the gland is imperative to gynecomastia surgery and is often impossible with liposuction alone.

What are the current trends for Gynecomastia surgery in Dallas?

According to The Aesthetic Society, liposuction is the most common cosmetic surgical procedure in men, with breast reduction treatment of gynecomastia as the second most common. More and more men are also choosing to combine their cosmetic surgeries.  The most common combinations are:

– Daddy do-over with male breast reduction combined with liposuction of the abdomen

– Daddy do over with male breast reduction combined with tummy tuck

– Male breast reduction combined with eyelid surgery (blepharoplasty)

– Male breast reduction combined with nose surgery (rhinoplasty)

These national trends are in keeping with the trends Dr. Azouz has seen in the Dallas/Fort Worth metroplex.

Is there a “right age” to have gynecomastia surgery?

There is no right age to address male breasts.  Dr. Azouz performs the surgery in preteens, teenagers and even in men who are in their sixties and seventies.  Most men who are older to report wishing that they had done it sooner.

How does gynecomastia surgery deal with loose skin after removal of the male breast?

Dr. Azouz uses specialized liposuction techniques including tumescent liposuction, power-assisted liposuction, and safe liposuction among other techniques to tighten the skin after gynecomastia surgery.  Some patients with gynecomastia and loose skin will require skin tightening procedures or breast lifts at a different time.  Dr. Azouz discusses the chance of needing a skin tightening operation in the consultations.

Do I Have Gynecomastia?

There are many reasons for Gynecomastia or enlarged male breasts. Some men are affected by Gynecomastia while they are teenagers, this phase of hormonal surges can cause the growth of male breast tissue. Other causes may be excess weight gain or the use of certain medications. Unfortunately, some patients are not able to get rid of Gynecomastia in adulthood, even though they exercise frequently.

Despite the many medications, hormones, drugs and supplements that are risk factors for Gynecomastia the most common cause is thought to be GENETIC.

The cause of Gynecomastia is usually a genetic predisposition and gynecomastia tends to run in families.  Pseudo gynecomastia can be caused by weight loss but most men will usually have a combination of gynecomastia and pseudogynecomastia.

Typically, gynecomastia is present in both breasts and forms a disc-like mound of tissue under the areolar complex. It is also possible for some to have unilateral or asymmetrical breasts.

Gynecomastia surgery: Five Post Operative Tips for your Recovery

Gynecomastia surgery also known as male breast reduction surgery can be life-changing cosmetic surgery for men with gynecomastia. The postoperative recovery is essential for the best results. During a consultation with Dr. Azouz, he will thoroughly explain everything you need to know about the recovery process, but just in case you’re curious, here are five postoperative gynecomastia surgery steps that Dr. Azouz recommends.

1) Wait to Return to Your Daily Exercise Routine

After gynecomastia surgery, you need to give yourself a week or two before getting back into your full daily aerobic exercise routine.  This means no strenuous physical activity or exercise, allowing your body time to completely heal. However, just a few days (frequently one to two) after your surgery you will be able to add in light daily activities such as working on the computer, desk work or business meetings.

2) Wear Your Compression for your Drainless Gynecomastia Surgery

After gynecomastia surgery Dr. Azouz requires you to wear compression garments. The reason for continuous compression of the surgical area is to decrease swelling and promote proper healing.  Dr. Azouz performs gynecomastia surgery without drains. During the healing process skin retraction is promoted by Dr. Azouz’s plastic surgery techniques and compression garments.

3) Good Posture Matters

Good posture is a key factor to the recovery of gynecomastia surgery. Hunching and slouching is commonly seen in gynecomastia patients as they subconsciously are trying to hide their unwanted gynecomastia.  After you’re surgery you will have nothing to hide, standing up straight with your shoulders rolled back will promoted the healing process and decrease future postural problems.

4) Tissue Massage at the right time

After the first three weeks of healing (or as recommended by Dr. Azouz), you should start massaging the surgical area to help soften scar tissue which helps the skin lay flat and evenly. Dr. Azouz will advise if this tissue massage can be done with your hands or a roller. The massage routine should be done several times a day during recovery as advised by your plastic surgeon Dr. Azouz.

5) Stay appropriately hydrated

Hydration is an important factor in the healing process. The recommended daily fluid intake is 2 liters for most patients.  Dr. Azouz will advise when a patient should increase or decrease their daily intake of fluid.  It is important to avoid salty or fried foods after gynecomastia surgery as they can cause fluid retention and extra swelling.  Saunas and hot tubs should also be avoided as they can lead to an increase in postoperative swelling.  Dr. Azouz guides his patients and releases them for a variety of activities including heavy exercise and use of pools, Jacuzzis and saunas.

Gynecomastia: Benefits of Male Breast Reduction

Gynecomastia, or enlarged breast tissue in men, can be caused by several factors ranging from genetic conditions to the use of certain drugs. Regardless of the condition’s cause, men dealing with gynecomastia frequently suffer from reduced self-esteem and wish for flattened, masculine chest contours. Male breast reduction, also known as gynecomastia surgery, is a procedure designed to decrease the size of the breasts and create a more masculine-appearing chest. Gynecomastia is more common than you might think, and male breast reduction has many benefits for the patients who choose this procedure.

Improved Male Chest Contours

In addition to reducing the size of the breasts, a male breast reduction can improve the contours of your chest overall. As a well-regarded gynecomastia surgeon, Dr. Azouz understands that in addition to removing excess tissue, the chest contours can be reshaped for a better result. This gives you a natural-looking, enhanced masculine appearance.

Reduced Breast Size for Overdeveloped Breasts

In men, the breasts can become overdeveloped for a variety of reasons. This includes a hormonal imbalance, which can be genetic or taking certain medications. When these are the cause of your gynecomastia, no amount of dieting or weight loss is likely to improve the appearance of your chest. Male breast reduction is the best way to permanently reduce the size of the breasts.

Reduced Excess Fat in Male Breasts

In some cases, the appearance of male breasts may be due simply to stubborn excess fat. This appearance can persist even if you are keeping up with a healthy lifestyle. Male breast reduction can help to reduce this excess fat, giving you the masculine appearance you’re looking for.

Reduced Excess Glandular Tissue in Male Breasts

Liposuction alone is often not enough to address gynecomastia, as excess glandular tissue may also be to blame for the enlarged breasts. In these cases, Dr. Azouz can carefully and effectively reduce this excess tissue using advanced surgical techniques.

Improved Confidence and Self Esteem

For male breast reduction patients, improved self-esteem is one of the major benefits of the surgery. Gynecomastia is typically not a health concern, but the condition can be a source of embarrassment and self-consciousness for many. Male breast reduction can help restore self-confidence.

Cost of Male Breast Reduction

The cost of gynecomastia procedure depends on many factors and can only be evaluated after a consultation with Dr. Azouz. If you would like more specific details regarding all the fees involved in a cosmetic surgery procedure, contact the office of Dr. Azouz today and his helpful office staff will be able to assist you.

Combination Procedures

Liposuction is a very common procedure to combine with male breast reduction surgery. Liposuction assists in the removal of breast tissue and can be used on other areas of the upper body like the flanks, abdomen, and hips.

Male pectoral implants can be combined with male breast reduction as well. Implants are commonly used for patients with Poland Syndrome and for those with significant asymmetry in their chests.

Gynecomastia: Dallas Plastic Surgeon Dr. David Azouz Busts Several Gynecomastia Myths

Gynecomastia is a common condition that can be disturbing to many men.  Unfortunately, many myths still persist today that cause men to be discouraged when seeking treatment for gynecomastia.

Male Breast Myths

Myth: ‘Pseudogynecomastia, Man boobs, Male Breasts, Gynecomastia are mutually exclusive (you have one or the other)

Dr. Azouz:  Most men have a combination of fatty tissue and gynecomastia.  It is not uncommon for men to have massive weight loss and have true gynecomastia (pure breast tissue).  Similarly, men who are at their ideal weight may have significant fatty tissue in their chest.  Most men will have a combination of gynecomastia (breast tissue) and fatty tissue.

Myth: ‘Nipple Size is independent of gynecomastia’

Dr. Azouz: Enlarged male nipples are most often associated with underlying gynecomastia.  Patients undergoing gynecomastia surgery can often expect their nipple-areola to reduce by half to one-third of the size.

Myth:  ‘You will have Drains with Gynecomastia Surgery’

Dr. Azouz: We do not use drains in gynecomastia surgery.  This allows a more comfortable recovery without additional scarring from drain sites.

Scar with Gynecomastia Surgery “The scar is most often imperceptible and camouflaged in the areola.”

Myth: Scarring with Gynecomastia surgery: ‘scars are multiple, large and visible’

Dr. Azouz: We only place one scar on each breast in our procedure.  The scar is most often imperceptible and camouflaged in the areola.

Myth: ‘Older men are not candidates for gynecomastia surgery’

Dr. Azouz: Older men can have wonderful results with gynecomastia surgery in Dallas.  We operate on men in their 70s and 80s routinely and have many older patients who are very happy after male breast reduction surgery.

Myth: ’Teenagers and pre-teens are not candidates for gynecomastia surgery’

Dr. Azouz: Young adults and teens can have wonderful results with gynecomastia surgery in Dallas.  We operate on boys as young as 10 and 12.  There are patients who have severe gynecomastia even before they are teenagers.  Gynecomastia surgery can be performed in even growing boys.

Myth: ’Male breast reduction is very painful’

Dr. Azouz: Many of our patients do not take any pain pills after male breast reduction surgery.  Some patients only take Tylenol and Advil.  It is rare for a patient to ever need a refill of their pain prescription after surgery.
